Applications for Concealed Carry Permits are now completed online. Approved applications are due for renewal every two (2) years. Renewal applications can be submitted up to four (4) months before the expiration date. Concealed Carry Permit applicants need to have been fingerprinted for firearms purposes in New Jersey and assigned an SBI number before they can apply for a Concealed Carry Permit. If you have never been fingerprinted for firearms purposes in New Jersey before, you must start with an application for an Initial Firearms I.D. card by visiting https://www.njportal.com/NJSP/fars.
STEP 1
First, write down the Middlesex Borough Police Department’s ORI Number before proceeding to step 2. The applicant MUST live in the Borough of Middlesex to use this ORI Number. If you do not live in the Borough of Middlesex, please contact your local police agency for their ORI Number. Middlesex Borough Police Department’s ORI Number is: NJ0121100.

STEP 3
Complete the online application following the instructions provided. You may complete the application using a smartphone, mobile device, laptop or desktop computer. References cannot be a family member by blood or by law and you must have known each reference for a minimum of 3 years.
Please review the entire application for accuracy prior to submitting it. If your application is submitted with errors or information was omitted, the Middlesex Borough Police Department does not have the ability to make corrections or add additional information and you will be required to submit a new application.
STEP 4
A $150.00 payment is owed to the Borough of Middlesex. Only cash (exact change) or money order (payable to the “Borough of Middlesex”) will be accepted. Please email firearms@middlesexpd.com to schedule an appointment to drop off the payment at Middlesex Police Headquarters.
